Youth Player Registration Forms
Indiana Soccer player registration is completed by member clubs using the GotSport registration platform. Each club maintains their own player registration form to collect the data before submitting to Indiana Soccer through GotSport.

Out of State Player Permission
Indiana Soccer no longer requires Indiana residents to obtain approval to participate in soccer activities in another state. However, to ensure accurate record-keeping, we ask that all players complete the form below if they choose to play outside of Indiana. If you have any questions, please contact Amber Cremeens.
